Preparedness Survey

As a fallout from our last meeting in Southern Alberta we discussed the use of a survey to identify gaps in our preparedness and to gauge our readiness as a community. Consequently, we have created the following anonymous and voluntary survey to help gather this data. No personal information is collected as part of this survey.

The intent is to gather this data for a period of 10 to 14 days starting today (November 1, 2012), at which point the survey will be closed and the data collated and the results shared.

Tried to have a look at both pgs - but it only allowed me the first page until I filled it out. I wanted to look at it to see if it could be adapted for Australia.
Is there a way I can look at both pgs.

I would suggest that you just provide any answer for each question on the first page to advance to the second/last page. Unfortunately, I had to make the questions mandatory so you won't be able to see it otherwise; however, you don't have to submit the form you can just cancel at anytime prior to clicking 'Finish Survey'. Hope that helps.

Hoping to receive some more participants in the survey. I think we can gather some very interesting metrics in terms of identifying gaps. I'm also interested in any feedback on the survey itself so that we ensure we cover all core competencies.
